Ota was last seen in Honolulu, Hawaii on March 3, 1979. He was seen at about 5:30 a.m. sitting in his blue 1976 Honda Accord coupe with Hawaii license plate 8B-1844 in the driveway of his Hawaii Kai home; the car has never been recovered. He was reportedly seen again in the days afterward, the last time on March 12, 1979, and has not been seen or heard from since. He was wearing a white t-shirt with Kuilima lettering, tan pants and slippers. His case remains unsolved.
